Suicide remains a leading cause of death among adolescents and transitional-age youth in the United States, with the period following psychiatric hospitalization representing a particularly high-risk time for suicide attempts and rehospitalization. Although inpatient psychiatric teams routinely assess suicidal ideation, intent, prior attempts, and protective factors, emerging literature suggests that suicide risk among Latino/a/x youth is also significantly shaped by broader social, cultural, and structural influences that may not be consistently incorporated into standard clinical assessments. Suárez-Brito et al. found that acculturation-related stressors—including family conflict, discrimination, and identity-related stress—are associated with suicidal ideation and suicide attempts among Latina adolescents, underscoring the importance of culturally informed psychosocial evaluation in suicide risk assessment.
Current practice gaps also include inconsistent assessment of social determinants of health and barriers to treatment engagement during discharge planning for transitional-age youth. Factors such as insurance instability, transportation barriers, language discordance, financial stress, limited outpatient availability, and lack of culturally responsive mental health services can significantly affect continuity of care following hospitalization. Stafford et al. identified persistent disparities in access and retention in mental health care among Latino adolescents and young adults, suggesting that suicide risk assessment should extend beyond acute symptom evaluation to include a patient's ability to realistically access and engage with outpatient care after discharge.
Additional practice gaps exist in addressing how cultural identity, family systems, stigma surrounding mental illness, and structural inequities influence psychiatric outcomes among Latino/a/x youth. Martínez et al. highlighted the importance of culturally responsive suicide prevention strategies, including family-centered interventions, enhanced care coordination, and community-based supports. The case discussed this month highlights the need for psychiatric clinicians to incorporate culturally informed assessment, social determinants of health, and individualized barriers to care into suicide risk formulation and discharge planning for transitional-age patients with recurrent suicidal behavior.
