The NEJM Case Records of the MGH presents a clinicopathological conference: A man with history of oral squamous cell carcinoma, traumatic brain injury, chronic pain, accidental opiate overdose, and adjustment disorder was evaluated because of irritability, confusion, and bizarre behavior. A diagnostic test was performed.
