Clinical practices in health care systems must have review processes and procedures in place to limit adverse outcomes by identifying practice gaps and improving the existing knowledge of physicians. One such process is the morbidity and mortality conference, which aims to focus on the review of adverse outcomes to identify system or practice errors and improve patient safety. This is achieved through structured case-based discussion of specific patients, root-cause analysis, and collaborative discussion among physicians. One consistent and significant focus for the psychiatric field is suicide prevention. Suicide and suicidal behavior are important public health problems, with increasing suicide deaths in the United States. National rates of suicide have risen steadily, including the rates of fatal drug overdoses, which have risen 4.4-fold from 1999 to 2015. Risk factors contributing to suicide attempts may differ in various environments, and one consideration is risk in the hospital or medical setting.
A recent observational review of reports of suicide or suicide attempts on hospital grounds or clinic areas identified primary root causes as breakdowns in communication, the need for improved psychiatric and medical treatment of suicidal patients, and problems with the physical environment. In this talk, we will review a case with this consideration and discuss contributors of risk, various phenotypes of suicide or suicidal behaviors, and interventions, including universal, selective, and other indicated interventions. The aim would be to expand existing knowledge for physicians on this topic and facilitate further consideration of potential interventions for suicide prevention in clinical areas.
